2020 Online Enrolment Form

This is the 2020 class enrolment form for Kiama Actors Studio. If under the age of 18 this form must be filled by a parent/guardian. This form must be completed prior to your first class.

Enrolment into your preferred class times is not guaranteed until confirmation by studio director. This could be due to the class already being full or being not appropriate for your age/experience. It is advised to keep in mind a back up class incase placement is not available for your first preference.

All sections of this form are required.

In the case of an emergency I give permission for Kiama Actors Studio staff to call an ambulance and understand that I must meet any costs incurred. I am aware that Kiama Actors Studio will make every effort to contact me or an emergency contact listed on this form.

I hereby grant the permission for films, video, and/or audio tape recordings and photographs to be taken during Kiama Actors Studio activities. I understand this media will be produced and used for educational purposes. I authorize Kiama Actors Studio to use my photo or video on it’s website, and or other related promotional material.

I understand that it is my responsibility to contact Kiama Actors Studio 24hours in advanced if unable to make class.

I understand that KAS will communicate with me mostly via email or text and I undertake to inform KAS if my email address or any other details should change.

I understand that enrolment is a term commitment. I will attend all classes unless sick/emergency circumstance.
